What this dataset is

Skilled nursing facilities (SNFs / nursing homes) have a national CMS citation spine — the Provider Data Catalog health deficiency extract plus survey summary and official Statement of Deficiencies (2567) text. Grain is one health citation per row: CCN, survey date, F-tag (or other deficiency prefix + tag), CMS scope/severity letter A–L, correction status, and standard / complaint / infection-control flags.

We load and operate that spine in SQL (DimFacility, FactHealthCitation, FactSurveySummary, FactSurveyNarrative, DimScopeSeverity) with fail-closed validation, R12 views, and a citation↔narrative join on CCN + survey date + tag for the current CMS SOD package (full match on the July 2026 production load). Internal RiskWeight on A–L is prioritization only — not a CMS Five-Star Health Inspection score. Published star ratings may appear as facility context only.

What you license is the operated product: normalized national tables, rolling R12 operator metrics, SOD narrative lineage, sample extracts, and a key-based API after purchase. This is not assisted living (ALF is a separate state-adapter spine). It is not SNF Facility Operator Performance (census / finance / staffing demo). CMS publishes the source files; the product is the maintained model, joins, honesty rules, and access package — not a one-time raw dump.

Spine snapshot

Figures as of our July 2026 production load of the CMS national nursing home extract (Provider Data Catalog processing date 1 Jun 2026). Rolling 12-month (R12) metrics use surveys through the latest survey date in that extract — 20 May 2026. Counts move with each CMS refresh.

Coverage honesty

Unlike ALF, nursing home health citations are a national CMS bulk extract — there is no A/B/F FOIA ladder for the core fact. Honesty here means: batch and as-of dates are explicit, R12 is survey-date based (not calendar “today”), open-like is a documented mapping of multi-value CMS correction language, and we never claim Five-Star equivalence for internal risk weights.

Sample extracts (proof of grain)

Capped slices from the same national CMS extract we loaded in July 2026 — for column inspection, not a full license. Minnesota is a sample geography for facilities and R12 citations; tags and S/S are national reference slices.
